Baptist Memorial Health Care is a network of hospitals throughout the Mid-South that includes 22 affiliate hospitals across 3 states and facilities that encompass an array of home care and hospice agencies, hospitals, minor medical clinics, behavioral health programs and a network of surgery, rehabilitation and other outpatient centers. The vision of the Palliative Medicine Service is to build the Specialty Palliative Medicine inpatient consult service under the hospitalist umbrella. Our goal is to support our hospitalist team on a consultant basis for inpatient work, and eventually grow outpatient clinics, and expand home-based palliative medicine visits among Baptist patients. Baptist works with a separate, established hospice group that will support the Palliative Medicine Service. We want to reach all patients who are diagnosed with a serious or life-limiting condition at the beginning of their journey and at any other stage of their disease process.

PRACTICE OPPORTUNITY:

• Build the foundation for our first Palliative Care team in Jackson, with the long-term goal to become one of 9 Palliative Care Leadership Centers
• Initially, our Palliative Care team will consist of:
• 1 Board Certified Palliative Medicine Physician
• 1 Certified Nurse Practitioner
• 1 Dedicated Registered Nurse
• 1 Dedicated Clinical Social Worker
• 1 Dedicated Chaplain
• Participate in expanding to outpatient palliative clinics
• Participate in expanding to palliative home-based setting
• Help develop Palliative Medicine Inpatient Service Policies and Protocols

• Board Eligible/Board Certified in Palliative Medicine
