Summary Performs intake processes for patients entering programs within the Malignant Hematology and Transplant (MHT) setting. Such programs may include, malignant hematology, autologous or allogeneic stem cell transplant, cellular therapy (CAR T-cell) and gene therapy. Specific tasks include receiving new referrals, verifying insurance, completing clinical history intake to satisfy requirements of the given program, communicating with providers, transplant coordinators, clinic staff and patients about the next steps in the care process. This position includes tracking patients in collaboration with clinicians and transplant coordinators as they move within the MHT Program. Coordination of care before/during clinic visits and participation in programmatic meetings is also required. This position will interface with other team members, including transplant coordinators, financial coordinator, clinicians front desk staff and external customers to satisfy all program goals and requirements. Performs other duties as assigned.
Responsibilities - Coordinate new referrals and verify insurance
- Obtain prior authorization for transplant/cellular therapy related care (evaluation testing, line placement, inpatient admission, discharge meds post-transplant)
- Prepare itineraries for transplant protocols, testing and appointments and send to patient
- Assist with coordination of post-transplant/cellular therapy care including scheduling routine visits and testing per post-transplant protocols and communicate with patients
- Attend weekly multi-disciplinary MHT Patient Conference
- Complete Malignant Hematology & Transplant Program-specific clinical intake
- Coordination, documentation and tracking of care processes in EPIC
- Communication with providers about Malignant Hematology & Transplant Program-directed care
- Communication with patients about Malignant Hematology & Transplant Program-directed care
- Triage calls - including escalation of urgent calls to appropriate provider
- Supports other team members with program goals
- Completes assigned goals
Requirements, Preferences and Experience Education Preferred: Associates Degree
Minimum: Skill in communicating clearly and effectively using standard English in written, oral, and verbal format to achieve high productivity and efficiency. Skill to write legibly and record information accurately as necessary to perform job duties.
Experience Preferred: 3 Years experience in onocology clinical work
Minimum: 3 Years medical office experience. Knowledge of medical terminology or procedures.
