Nurse-Navigator RN - TC Multid Clinics BMH Tipton

Summary

Provides the patients identified by the Lung Nodule Program with services including: support at diagnosis, patient education, coordination of care during diagnosis and nutritional counseling. Offers support groups for patients, families, and significant others. Coordinates appropriate referrals and markets services to community. Performs other duties as assigned. Anticipated case volume or navigation is ~150 cases per month for 2 navigators

Responsibilities

• Initiates patient contact upon obtaining referral from physicians.
• Coordinates access to services and education for the cancer patient.
• Participates in activities to improve health care delivery.
• Provides patient, family, and/or staff teaching/education.
• Coordinates marketing of cancer services.
• Provides leadership.
• Participates in ongoing educational activities.
• Completes assigned goals.

Requirements, Preferences and Experience

Education

Minimum: BSN required

Experience

Preferred: Preferred: Experience in oncology nursing

Minimum: 3 years clinical nursing practice in oncology nursing required.

Licensure, Registration, Certification

Minimum: RN, AHA BLS

Special Skills

Minimum: Broad knowledge of community services, health concepts and adult education.
