Coordinator -Behavioral Health Integration LSW

Summary

This position provides behavioral health care services for BMG ambulatory patients via scheduled visits or crisis visits.

Responsibilities

Provides billable and non-billable mental health services. Works professionally to coordinate services and therapy for individuals and families. Coordinates referrals to community and social services, including follow up services. Provides interventions for crisis visits and coordinates any necessary services based upon those visits. Assists patients with social needs (transportation, food insecurities, nursing home or assisted living placements, additional in-home personal care resources, home health or hospice conversations, etc.). Collaborates with the patient's care team members regarding behavioral health and social needs. Complies with policies that support patients' fundamental rights to considerate care, protect their personal dignity and respect their cultural, psychosocial and spiritual values in an effort to meet patients' care needs and preferences.

Requirements, Preferences and Experience

Master's in Social Work.

Licensed in Clinical Social Work.

Experience in an outpatient behavioral health setting preferred.
