Nurse-Navigator RN - BMG Clinic, Cardiovascular & Thoracic Surgery Memphis

Summary

Provides the patients identified as part of the Cardiovascular Center of Excellence with services including support throughout the entire continuum of care; providing guidance in coordination of care. Offers support to patients, families and significant others. Coordinates appropriate referrals, facilitates scheduling, communicates with patients, families significant other and referring physicians. Tracks individual patient's progress along care continuum and assists in overcoming barriers to care. Connects patient to support services. Performs other duties as assigned.

Responsibilities

• Initiates patient contact upon obtaining referral from physicians.
• Coordinates access to services and education.
• Participates in activities to improve health care delivery.
• Provides patient, family, and/or staff teaching/education.
• Participates in ongoing educational activities.

Requirements, Preferences and Experience

Education

Preferred: MSN preferred or clinical nurse leader, Masters level advance training

Minimum: BSN required

Experience

Preferred: 5 years of experience in general and/or surgical cardiovascular nursing preferred.

Minimum: 3 years clinical nursing practice in general and/or surgical cardiovascularnursing required.

Licensure, Registration, Certification

Minimum: RN

Training

Preferred: Preferred coaching and training experience.
