Summary

The Health Navigator will coordinate with appropriate staff and insurance payers to ensure smooth patient flow through the healthcare system.

Responsibilities

• Manages a caseload of patients
• Acts as a liaison between the patient, physician, and other medical providers and insurance companies to ensure quality care for patients
• Coordinates with appropriate staff and insurance payers to ensure smooth patient flow through the healthcare system
• Completes all assigned goals
• Performs all other duties as assigned

Requirements, Preferences and Experience

Education

Minimum: Licensed LPN in the state of Mississippi

Experience

Preferred: Care coordination nursing experience

Minimum: 1 year nursing experience

Licensure, Registration, Certification

Preferred: RN

Minimum: LPN

Special Skills

Minimum: Strong analytical and data management skills, comfortable seeking direction and assistance from appropriate resources, ability to communicate effectively verbally and written, proficient ability to use computer and electronic medical record

Training

Minimum: Excellent interpersonal skills with the ability to effectively communicate, both verbally anbd written, with patients, physicans and office staff to provide optimal levels of patient care. Skill and proficiency in applying the highly technical principles, concepts and techniques which are central to referral coordination and clinical protocol and guidelines.
