Summary Summary Provides nutritional support to participants/members in the Baptist Health and Wellbeing program assisting in the development of nutritional strategies to improve the health status of defined populations. Serves as a nutritional advisor, advocate, and educator to program participants in the achievement of specific chronic disease management objectives to improve health outcomes and status through nutritional assessment, consultation, and education. Utilizes a number of population health management tools and analytics to design education and resources so that program members are actively involved in the management of their own health and wellbeing. Works in coordination with the Baptist's Health and Wellbeing professional team and other health care providers, as needed, to monitor care coordination efforts for both incremental and long-term improvements. Performs other duties as assigned.
Responsibilities • Conducts thorough nutritional assessments on program participants to identify strategies in the prevention, detection, and/or treatment of chronic health issues.
• Develops a customized care plan to meet assessed nutritional needs.
• Designs and conducts educational offerings on pertinent nutritional topics linking healthy choices to increasing health outcomes, enhancing quality of life, preventing/decreasing chronic health diseases, and learning/identifying tools and resources to better manage their own nutritional health decisions.
• Works closely with the Health and Wellbeing team to implement outcome based quality measures/metrics and performance improvement approaches to monitor and gauge health improvements for defined populations.
• Completes assigned goals.
Requirements, Preferences and Experience- Bachelor's Degree in Dietetics, Clinical Nutrition, Nutrition Science or a related field from an accredited college or university by the Accreditation Council for Education in Nutrition and Dietetics required (ASCEND).
- Master's Degree from an accredited dietetics program and/or experience as a RD or RDN preferred.
- 2 years or experience as a RD or RDN required
- Excellent organizational and interpersonal skills and ability to develop and foster collaborative partnerships with a variety of key stakeholders. Possesses knowledge of population health management tools to design care plans, provide counseling, and/or education, and monitor health improvements for program participants. Develops and conducts training and education to designated groups to strengthen program offerings or community awareness. Effectively prioritizes in high volume business setting. Ability to travel as needed to Baptist entities system-wide required.
- Licensure in State of TN and upon hire will obtain licensure in MS and AR required.
